Payment Methods
Check, Credit/Debit, Cash (If cash, please bring exact amount -- the office does not keep change on the premises.) Billing is available for Motor Vehicle Insurance with a doctor's referral. No other insurance is billed at this time.
*All payments are to be made at the time of service unless arrangements have been made beforehand. The above prices reflect a time of service discount based upon a savings of administrative costs. Normal rates are $20.25 per unit.
Cancellation Policy: If you need to cancel an appointment, please give a minimum of 24 hours notice and you will not be charged for the cancellation. Otherwise a standard $25 administration fee will be charged. No shows or repeated last minute cancellations may be charged the full appointment fee.
